Here’s how you can start taking advantage of your membership: 

1. Download our communication app
   Click the button above or click here to sign up for Spruce, our secure messaging and telehealth platform. Now you are ready to connect with me via text/virtual visits at your convenience!  You may also call the practice at 317-763-2131

2. Schedule an appointment
   You may request an appointment by:

-Texting through Spruce 

-Emailing info@westfieldpremier.com

-Calling 317-763-2131

3. Medical History Form:
   Once you schedule your first appointment, you will receive an email requesting you complete your medical history form electronically prior to your visit.  

4. Past Medical Records:
   If you would like us to obtain your past medical records, please submit the Record Request Form to your past provider.

5. Access your health information:
   After your first visit, you will receive a link to access the Elation patient portal.
